Debriefing and journaling questions (Terra Nova Pilgrim House)

Yesterday on my way to the airport I met this lovely lady from Ireland and she showed me the 'Debriefing and journaling questions' she got at the Terra Nova Pilgrim House.

Does anybody have this form as well and would be willing to send it to me by mail ?
